Chemical Skin Peels Defined and Benefits
By David Maillie A chemical skin peel is a popular procedure used to treat various skin problems and conditions from acne, age spots, wrinkles and even some cancerous and precancerous growths. A caustic acid or substance is applied to the skin to burn and usually slough off or remove the outer layers of the skin (epidermis) and allow new skin to grow. Some chemical peels may go deeper into the dermis (next layer of skin). The purpose of this chemical peeling is to remove damaged, blemished, sun-damaged, freckled skin and replace it with younger, healthier skin. After healing the skin will be tighter, thicker (due to increased collagen production an added benefit of the stronger peels). Upon healing the skin may appear fuller, smoother with smaller pores and with less wrinkles and fine lines. Basically ones skin will look rejuvenated and more youthful following a chemical peel. There are many types and varieties of these chemical peels. They are usually grouped and defined by their relative strength and depth of the peel that is achieved. There are basically four groups, the light peels (AHA), light oil-based peels (BHA), medium peels (TCA), and deep peels (Phenol). AHA (Alpha Hydroxy Acids) - These are the mildest of the peels. They are usually sugar derived hence their names, glycolic, malic, lactic and other fruit acids. These peels are usually performed by a dermatologist or esthetician and have minimal healing or downtime. Downtime is referred to as the time when the skin is healing and may be uncomfortable to be viewed in public due to peeling and possibly discolored dead skin that is usually associated with a deeper peel. These lighter peels are usually done on an outpatient basis and are commonly referred to as a 'lunch time' peel. An occlusive dressing similar to plastic wrap or layering (more than one application at one time) may be used to drive the acid in further and result in a deeper peel. They are effective for light skin pigmentation problems, reduction of oil produced by overactive sebaceous glands, sun spots, light sun damage and other minor skin problems. Usually multiple treatments are required to have the desired effect as it is not a deep peel. AHAs are easily dissolved in water and are prepared either as a liquid or a cream. Glycolic acid is the most popular and extensively used of these peels. BHA (Beta Hydroxy Acids) The main difference between these acids and alpha hydroxyl acids (AHA) is that BHAs are only oil soluble. There is basically only one BHA, salicylic acid. This unique feature of being oil soluble makes this acid very effective in cleaning and removing bacteria, dead skin cells and sebum oil from deep inside the pores. Because of this solubility difference, salicylic acid is better suited for use on oily, irritated skin with postules, blackheads and whiteheads. It is available in numerous forms, cream, gel, lotions, shampoo and topical solutions. The latter two forms are primarily for treating the scalp as salicylic acid has also been found effective in the treatment of dandruff, psoriasis, and dermatitis of the skin and scalp. Same as with AHA peels, occlusive dressings and/or layering may be used to drive the acid in farther. TCA (Trichloroacetic Acid) TCA or trichloroacetic acid peels are deeper medium depth peels. TCA is a more effective and longer lasting peel than the sugar and citric peels as it goes deeper into the dermis and can therefore treat more skin ailments, conditions and problems. A TCA peel does not require layering or occlusive dressings (methods used to drive AHA and BHA peels deeper). TCA, being a deeper peel, is effective on more skin conditions and problems. Basically a TCA peel has the benefits of both AHA and BHA peels along with deeper penetration and longer lasting effects. Healing or downtime tends to be longer as much as several weeks or more depending on the depth achieved. Peeling is more severe and the results more dramatic. TCA peels are routinely used by dermatologist and plastic surgeons in office as recommended treatments for all stages of acne including acne vulgaris. Numerous clinical studies have found to be every bit as effective as Accutane without any of the side effects (Accutane has been in the news lately for some rather severe potential side effects). TCA peels have also been proven to remove and treat freckles, years of sun damage, bad pigmentation, scars, loose skin and wrinkles and fine lines. TCA has recently even been used by doctors to treat, remove and lighten tattoos. Phenol Peels. Phenol peels are the deepest chemical skin peels. They commonly result in a permanent bleaching pigmentation effect as they reach deeply through the dermis.. Phenol is toxic when absorbed through the skin and can effect heart rate, function and cause arrythmias, so a phenol peel is always done in a doctors care and with anesthesia as it is the deepest and most painful of the peels. It is a last resort peel due to its harshness and bleaching properties. Phenol is rarely performed nowadays due to its toxicity and the advent of safer medium depth peels like the TCA peel. Phenol peels have a long healing process and can require extensive care. There are various derivatives of each of these 4 groups, but basically these are the various chemical skin peelsl. Clinical studies published in the Annals of Plastic Surgery and elsewhere show the TCA peels are the most effective of the peels on the most skin conditions and problems. Regardless of the peel used, maximum sun protection must be used to protect the new skin from harmful UV rays that can cause cancer in unprotected skin. A high SPF (sun protection factor) sun block and protective attire (hats) are recommended. What are the Best Roles for Western and Alternative Medicine?
By Daryl Kulak I think it's very confusing for patients to decide what the best roles are for Western medicine and alternative medicine. In this article, I'll try to clear some of that confusion with a few simple principles. I can tell you how I view the difference between the two systems of medicine and you can decide what to do for yourself and your family. First, a short explanation of alternative medicine. It is often also called complementary medicine, integrative medicine, wellness or holistic health. Alternative medicine is a catch-all term that includes many healing methods from many countries and cultures. From India it includes yoga, ayurveda, chakra healing, and herbal medicine. From China it includes tai chi, chi gong, acupuncture, acupressure, massage and another set of herbal medicines. From North America it includes homeopathy, naturopathy, massage therapy, reflexology and many other types of therapies. By Western medicine, I mean the usual type of medicine used here in the U.S. and Canada. Pharmaceutical drugs, antibiotics and surgery are the most commonly prescribed treatments under Western medicine. The name alternative medicine makes it sound like these therapies should be used instead of Western medicine. But this really isn't the case. Hardly anyone ignores Western medicine and just focuses on alternative medicine. My own approach can be summarized in four principles:
Use the safest, cheapest, effective remedy first.
Use alternative medicine for chronic or lifestyle ailments.
Use alternative medicine for prevention.
Use Western medicine for acute and life-threatening ailments.
Again, this is my way of looking at alternative and Western medicine. You need to decide on your own approach. Using the safest, cheapest, effective remedy first is sensible, I think. Use safe, cheap remedies with no side effects first, then progress to stronger, more dangerous, more expensive treatments later. A good example of this is when my wife's allergies come on in springtime. She starts by taking an aromatherapy combination that she rubs on her nose and forehead. Most of the time, this works. But when it doesn't, she moves on to a homeopathic remedy for allergies. Usually, that will solve whatever problem the aromatherapy couldn't help. But if neither one works, she moves on to an off-the-shelf drug for allergies. And that usually fixes whatever symptoms the first two couldn't work on. I suppose if she was still suffering after that, she might go to the doctor and get a very strong prescription treatment. That hasn't happened yet, thankfully. Why not just take the over-the-counter drug first? Because it has bad side-effects. It makes her sleepy, it dries out her nose making it bleed, and lots of other bad stuff. And it is costly. But that's worth it if she's already tried the other safer, cheaper remedies. In another example, when I have a back problem, I'll go to a massage therapist, rolfer or Feldenkrais practitioner. They can usually help me. If not, I go to a chiropractor. Chiropractors are great for relieving back pain. If that doesn't help, then I'll think about taking off-the-shelf pain medication. If that isn't strong enough, I go to the doctor to get a prescription drug that can knock the pain out while I have the bodyworkers help me with the actual problem in my back. My second principle is that I use alternative medicine for the everyday problems and the chronic problems. By chronic, I mean the problems that linger day-after-day, week-after-week. Alternative medicine is well-known for helping people with the chronic ailments like diabetes, chronic fatigue syndrome, multiple sclerosis, etc. Chronic problems can also be less severe like back pain, soreness, obesity, chattering mind, etc. Western medicine seems clueless when it comes to these problems. Third, I use alternative medicine for wellness and prevention. I go to see a massage therapist even when I don't have any problems, just to help me stay healthy. I go to see a naturopath as a checkup every six months or so to make sure I'm on the right track and to report any seemingly small symptoms that may turn into something worse down the road. Fourth, I definitely turn to Western medicine for acute or life-threatening problems. If my appendix starts hurt, I'm not going to see a massage therapist or reflexologist. I'm going to trot over to the hospital emergency room. If I've been involved in a car accident and my legs got crushed, I want the best surgeons to put me back together. I follow these principles, not just because they are healthier, less risky and more prevention-oriented, but also because of another consideration. Cost. Western medicine, especially when used as a first resort, is just too expensive for me. My health insurance policy has a high deductible, because my wife and I are both self-employed and we cannot afford thousands of dollars a month for health insurance. So I want the best possible healthcare for the best price. And that seems to be alternative medicine more often than not. What Do My Lab Results Mean?

I found that to be true in my own health as well. When I first started on my path toward becoming a naturopath, I knew I had pre-cervical cancer. I was depressed and wanted to die. Yet, only one liver enzyme was a bit out of whack when a nurse drew my blood for a life insurance policy. How could this be? And what surprised me most was how normal the rest of my results were. Well, I've corrected all those abnormal results and have my life back. I also have a whole new bag of tricks at my disposal. Another thing Ive found over the years was that my clients had no idea what their lab results meant (or their pet's lab results), what a "rule out" was, or what they could do naturally to repair the abnormal values. Everything seemed so complicated for them, and by not understanding the results (often people in the medical profession use big words that most people cant understand) this disempowered my clients. Because I saw a need for this information, I sat down and compiled a handout that explains lab tests in lay terms, but there are some things you should know: 1. Reference "normal" ranges and unit measurements can vary from lab to lab (sometimes up to 30% difference). 2. Other factors influencing test results include, dietetic preferences, sex, age, race, species, menstrual cycles, amount of exercise, use of non-prescription drugs (aspirin, cold medications, vitamins, etc.), prescription drugs, alcohol consumption, collection and handling of the specimen. 3. For best comparisons of lab results, tests should be done by the same laboratory. 4. Always use the normal ranges printed on the lab report of that particular sample. 5. Blood tests can have false positives and negatives. If the blood cells have not been handled properly, they break. If components to the test are expired, they may not work correctly. If the temperature is wrong, this may also skew the results. And what if there is a new person in the lab who doesn't even do the test correctly? Would you trust those results? When in doubt, you may want to repeat certain tests. 6. Interpretation of blood tests takes knowledge of the underlying disease process and experience. If your lab results are outside the normal range, we suggest that you discuss them with your doctor. Often it is not the recent result, but the change from a previous test that is most helpful in trying to place a diagnosis on a condition. 7. Some tests cost more than others. If you can help the doctor understand why a certain test may be important for you and they can justify it in their mind, they can justify it to your insurance company and you may get to your diagnosis quicker saving time, money, and aggravation. 8. All diseases have "rule outs." There are a number of things that might cause a high or low level in a blood test, and these need to be ruled out by other tests or observations or even by intuition and gut instinct. Some of the possibilities will have more weight and be more highly suspect because of your lifestyle habits, your environment, your age, and your history. For those diseases that are the most likely diagnosis, other tests may be included to rule that particular disease in or out depending on the results of these additional tests. This is much like putting a jigsaw puzzle together and searching for the exact placement of the pieces to the puzzle. Certain clues (pieces to the puzzle) may fit in one part of the puzzle or another if you just look at the color of the piece (or your symptoms in this case.) You just have to keep trying to see if the piece fits. The same goes with disease. If we are lucky, we match the piece of the puzzle with the correct name/diagnosis for the disease. Geothermal Projects in Indonesia
By Mohamad Mova Al 'Afghani Update on renewable energy investment in Indonesia. There is an interesting article in the Jakarta Post on Geothermal investment. Indonesia has developed only less than a thousand MW of geothermal power although it is estimated to have nearly one-third of the world's geothermal resources. A research from the ministry of mineral resources mentioned that there are 252 geothermal locations which have been identified along a volcanic belt extending from Sumatera, Java, Nusa Tenggara, Sulawesi until Maluku. These heat resources has the total potential of 27 GWe, which puts Indonesia as the biggest geothermal potential country in the world! A Jakarta Post op ed told that (i) tax uncertainties and (ii) legal uncertainties are among the frontlines of investment barriers. Geothermal investment is really a high risk-high return investment scheme which involves huge exploration and exploitation funds. It is never intended as a short term investment. Investors are afraid that their contracts are not being honoured. I'll give a little comment on the legal barriers. Generally contract observance remained high. Business partners will think twice before deciding to bailout from a contract since litigation costs are too high. The problem stems not from observance of the contract but mainly due to regional autonomy. How much share will the regions receive are the hot issue. There has been a case where Garut regional government requests Chevron and the Central Government to disburse concession money for their region. They even threatened to invalidate Article 41 of the Geothermal Law to the Constitutional Court. Article 41 governs a transitory provision which stipulates that all contracts prior to the entry into force of the Geothermal Law will remain valid. Under this transitory provision, regions don't receive any money. Of course, their attempts to invalidate the article are futile. It must also be noted that the 2003 Geothermal Law gave many power to regions in particular with respect to licensing. This is a good opportunity for investors as they can now deal directly with the regions. During the old days, people will have to deal with state owned enterprise such as PLN or Pertamina. However, it is necessary to make sure that both central and regional government approves or condone the projects. It wouldn't be good if after a few years of investing the license given by regional government is revoked by the central, or a license given by central government is revoked by the central. In any event, although the regions are granted more power, it is always important to asks some sort of condonation from the central government. There are steps that we can all take to make sure our skin looks healthy and vibrant and stays wrinkle-free for longer. All we've got to do is invest some time and care in looking after it. It is important to understand the basic function of our skin in order to look after it properly. The skin is our bodys largest organ and comprises three major layers: The Epidermis: This layer has five sub-layers; the outer layer consists of mainly dead cells which are continuously shed and replaced by cells beneath. The epidermis is also responsible for water proofing your skin, protecting your skin from potential invasion of microbes and generate new skin cells which move from the deep layer of the epidermis to the outer layer, where they are eventually shed. The Dermis: Is the middle layer of the skin and is composed of connective tissue containing collagen and elastin. The dermis is thick in the palms and soles and very thin in the eyelids. It contains blood vessels, nerves, glands, and hair follicles. The Subcutaneous layer: This layer consists of areolar and adipose tissue. Fibres from the dermis extend down into the subcutaneous layer and anchor the skin to it. The subcutaneous layer, in turn, attaches to underlying tissues and organs. Aging of the skin There are several factors influencing how quickly our skin ages, ranging from genetic factors, your natural skin type, as well as external factors such as exposure to sunlight, environmental factors and whether you smoke or not. In general, pale skins wrinkle faster than darker skins, which are protected by increased amounts of pigment and lipids. Another vital factor is, of course, our age. Our skin looks and functions very differently at 20 as opposed to when we are 60. Below is a skin care plan of how to look after your skin as the decades go by and how to adapt your skin care regime accordingly. Looking after your skin in your 20s Your 20s is a great time for your skin. You've left behind the adolescence acne and your skin has a radiant, youthful glow and the epidermis is well toned. However, this is not the time to be complacent. In your 20s skin cell renewal drops by up to 28 per cent, dead skin cells are not as easily shed and external factors are starting to have a greater impact, leaving your skin slightly duller.
